Six patients with purulent complications after allografting of the great arteries were provided a complex of treatment measures including radical surgical management of the purulent focus, additional methods for wound treatment, and plastic reconstruction of the paraprosthetic area by advancement flaps of the m.gracilia and m.sartorius. The conditions for our attempts to preserve an infected graft were its function maintenance, the absence of the signs of infection and incompetence of anastomoses as well as the absence of bacteriemia. The use of the aforesaid complex allowed the removal of the manifestations of infection and maintenance of the vascular graft in five of the six cases. The long-term results were followed up for the period as long as three years. No recurrence of suppuration was recorded. The complex offered by us can be used as one of the variants of the treatment policy, keeping strictly to the conditions indicated.

KEY WORDS: vascular graft infection, advancement muscle flaps.
