The aim of the review is to analyze the reported date on the use of radionuclide techtiques in the assessment of the cerebrovascular reserve (CVR), to describe CVR assessment techniques including the quantitative method, and to demonstrate the clinical significance of the test employed. The acetazolamide test is used most commonly. As a rule, the CVR in patients with stenosing atherosclerosis of the carotid arteries is decreased as compared to that normal subjectis. CVR measurement is used successfully for evaluation of collateral circulation competence and detection of hemodynamically significant asymtomatic stenoses, for estimation of the hemodynamic stroke risk as well as for objective identification and selection of patients requiring revascularization surgery and evaluation of the surgical effects of the given operation.
