Among 86 patients operated for aneurysm of thoracic aorta in Marfan or Ertheim syndromes, 10% had concomitant ischemic heart disease. Thus simultaneous reconstruction of ascending aorta and/or aortic arch was executed with aortocoronary bypass via median sternotomy. The paper describes original procedure of distal aortic and aortic isthmus prosthesis with autovein aortocoronary bypass of posterior-lateral branch of circumflex artery from descending aorta on working heart without introducing of extracorporeal circulation. This method was successfully used in clinical practice and demonstrated good long-term results.

KEY WORDS: aneurysm of thoracic aorta, ischemic heart disease, simultaneous surgical procedures.
